23 an hour SUMMARY The Care Coordinator is responsible for assisting parents/caregivers in connecting with community services, providing ongoing care coordination and follow-up and educating parents and the public on developmental milestones.
Required Education:
Bachelor's Degree in public relations, special education, early childhood development, social work, psychology or related health or human services field preferred.
Required Knowledge & Skills Successful completion of 2-1-1 Specialist Training Bilingual in English & Spanish highly desirable Ability to work effectively with people of diverse backgrounds Ability to work independently as well as collaboratively Ability to problem solve, plan, organize, and coordinate multiple work assignments with an attention to detail, and meet deadlines in a fast-paced environment.
Ability to communicate effectively verbally and in writing in a clear, concise and effective manner.
Computer literacy in software applications such as Microsoft Office Suite, word-processing, spreadsheets, and databases.
Knowledge of services supports and systems that serve children and families, particularly Florida's system for early childhood services.
Ability to establish and maintain effective working relationships with others.
Ability to work independently with limited supervision.
A strong sense of and respect for confidentiality involving clients, stakeholders, and fellow employees.
Primary Job Duties:
Administer screenings to children using the ASQ-3, ASQ-SE, MCHAT, and the Pediatric Symptom Checklist.
Assist families with navigating community resources and services including providing support, assessment, and follow up activities to families, healthcare providers, and community partners.
Develop in-depth knowledge of Help Me Grow and local systems and services focused on early childhood development and special health care needs, as well as the eligibility, referral, application processes for these and related services Develop and coordinate collaborative relationships with appropriate organizations and individuals, including 211 staff Respond to calls from parents, childcare providers, educators, health professionals and social service agencies about child development issues and services Provide crisis intervention when appropriate, assess and connect clients with services through referrals and conference calls, advocate as needed and provide care coordination and follow up Participate in the training and implantation of Parent Caf 's in collaboration with Family & Community Outreach Specialist and subcontract partners for family engagement activities.
Participate in local Help Me Grow meetings, community and physician outreach activities Participate in training and technical assistance related to Help Me Grow work including but not limited to intake and referrals, screenings, and outreach.
Use and update the STAR data system Complete documentation and reports of Help Me Grow families' outcomes as well as barriers, service gaps, and related challenges that families experience Other duties as assigned Job Type:
